Master Promissory Note - This guarantees that you will be responsible for any charges incurred by you while a student at MBU. This is not a payment plan and there is no charge associated with the document. Please complete (print) the (1) date, (2) top line Maker, (3) Maker name, address, city, state, zip, (4) social security number, (5) the bottom half. Only sign where it says Maker(s) signature on the top half. An MBU rep will sign the lower left hand corner. Your classes will not be moved to official status until this note is completed and received by the Business Office.
